The market is too pessimistic about recurring insurance profits after quality bonuses return. The new clinic partnership improves financing flexibility, but does not justify raising earnings estimates. I retain my $498.18 twelve-month target.
Key points
- 2.3% in 2028 is the individual Medicare Advantage pretax margin the price requires under my valuation.
- 3% in 2028 remains my forecast, excluding exceptional quality bonuses.
- $498.18 a share remains my October 2027 target; clinic financing adds no forecast earnings.
The call
The market allows for an incomplete recovery. At 16 times recurring annual earnings, today's price requires approximately a 2.3% individual Medicare Advantage pretax margin in 2028. Revenue has grown much faster than profit. The strong six-month stock advance supports the recovery story, but does not establish that the work is finished.
Premium growth still needs to become profit ($ billions)- Reported revenue
- Reported operating profit
SEC quarterly results, Q1 2024–Q2 2026; latest filing July 29, 2026. Profit uses left axis; seasonal.
I expect better insurance profits, not a financing windfall. Plan exits, benefit changes and restored quality bonuses support my 3% margin forecast. Bonuses return in 2028, not 2027. The market still lacks quantified evidence of next year's margin improvement; December's investor update and enrollment results should begin resolving that uncertainty.
Valuation
| My valuation bridge | Estimate |
|---|
| 2028 individual Medicare Advantage pretax profit | $3.54 billion |
| 2028 other net pretax profit | $1.55 billion |
| 2028 recurring earnings per share | $31.14 |
| Core earnings valuation | 15 times |
| Recovery-confidence premium | 1 additional times |
| October 2027 stock target | $498.18 |
Insurance profit assumes $118 billion of annual premiums, only modestly above first-half 2026's annualized level. Other profit includes CenterWell, remaining insurance and corporate interest; earnings allow for a 25% tax rate, $50 million of investment losses and 121 million diluted shares. The confidence premium disappears if recovery stalls; a 2% margin supports about $381 a share, making this a buy rather than a strong buy.
The target depends on keeping more of each premium dollarMy October 13, 2026 scenarios: 2028 individual Medicare Advantage pretax margin; all use 16× earnings.
Risks
- Medical inflation could outrun reimbursement; watch claims costs, since 2027 industry funding growth remains below Humana's current care-cost assumption.
- Benefit cuts could lose profitable members; watch early 2027 enrollment and retention after plan exits.
- Clinic financing could retain costly obligations; watch the replacement guarantee's value and cash requirements, not an accounting liability release.
What would change my mind
I would reconsider if 2027 guidance and enrollment show that margins cannot progress toward 3% in 2028 without substantially deeper membership losses.
