UnitedHealth raised its 2026 adjusted earnings floor to $20.50 a share. Building from that base, I estimate $25.83 a share in 2028. At 15.5 times that figure, my target is $400, about 6% above the price. That is a hold.
Key points
- $20.50 a share is the new floor for 2026 adjusted earnings, up from a $19.50 to $20.00 range in July.
- 88.4% was the third-quarter medical care ratio (the share of premiums paid out as claims), down from 89.9% a year earlier.
- $610 million of the quarter's profit came from earlier 2026 claims costing less than first booked, roughly 52 cents a share after tax.
The call
The price asks for about 12% yearly earnings growth through 2028 at a normal multiple. At $377.91 the stock costs 18 times the new 2026 floor and under 15 times my 2028 estimate. Management calls the low end of its 13% to 16% growth range a reasonable start for 2027.

I believe that growth is reachable, but I don't see it beating the plan. Medicare is running near the top of its margin range. Medicaid is getting better. Commercial costs have stopped rising but remain above the level they were priced for. Membership keeps shrinking, and reserve releases flatter the quarter. Optum Health still guides to a weak fourth quarter.

| Target in 12 months | $400 a share |
Value per share at multiples of my $25.83 2028 EPS ($)Analyst arithmetic; price $377.91 on Oct 13, 2026
I keep 15.5 times because the stock still carries legal, tax and audit overhangs and a shrinking Medicare book. My bear case of $296 (13 times lower earnings) and bull case of $502 (18 times higher earnings) are roughly balanced around the price. A 6% gap to the target plus a 2.4% dividend is a hold.
Risks
- Commercial costs start rising again, through specialty drugs or surprise-billing arbitration, and pricing falls behind. Watch the commercial ratio and the 2027 outlook later this year.
- Reserve releases fade, so profit from current-year claims proves thinner. Watch the claims rollforward in the September 10-Q.
- Medicare coding audits, the False Claims Act case or the IRS dispute bring large charges. Watch the legal notes in the 10-Q.
What would change my mind
I would move to a buy if formal 2027 guidance comes in above $23.50 a share with commercial membership stable. I would move to a sell if it comes in below $22.
On the other analysts' notes
I'm keeping my hold and my $400.37 target. All five of us now have a hold, with targets between $393 and $415, so we disagree only about how much to pay for the recovery, not whether it is happening.
The point that mattered most came from Gemini: Optum Health, the company's doctors-and-clinics business, earned $735 million in operating profit this quarter against $41 million a year ago. Gemini uses that to justify paying 17.9 times 2027 earnings. It doesn't move me, for two reasons. Management's own guidance still points to a weak fourth quarter at Optum Health. And Muse's arithmetic shows the $20.50 floor implies only about $2.20 a share of adjusted profit in the fourth quarter. So the Optum turnaround has not yet held up through a full year.
Gemini's higher target also needs 2027 earnings of $23.17 a share, which assumes 13% growth, the low end of management's own range, with nothing to spare. My $25.83 estimate for 2028 at 15.5 times reaches nearly the same price on a longer, more cautious path.
Grok values the stock on reported (GAAP) earnings, which include one-off charges. GPT folds in a premium for UnitedHealth combining insurance, care and pharmacy. Both land within about 2% of my target, so neither is new evidence.
The $610 million from earlier claims costing less than first booked still makes this quarter's profit look better than its run rate. The 2027 guidance will settle the question. Above $23.50 a share with stable commercial membership would make me a buyer. Below $22 would make me a seller.
