The market is about right about the recovery. Better government-insurance results and commercial repricing justify withdrawing my sell, but care-delivery profits still need rebuilding. My twelve-month target rises to $393.47.
Key points
- $21.00 a share of 2027 reported earnings supports today's price at my valuation.
- $21.86 a share is my rebuilt 2027 forecast after stronger insurance results.
- $393.47 a share is my October 2027 target, replacing my previous sell.
The call
The market now asks for achievable improvement. At 18 times annual earnings, today's price requires roughly 8% growth over the 2026 reported earnings floor. Medicare performance and commercial pricing make that attainable. Shares have nevertheless lagged the market over three and twelve months, so confidence remains fragile.

I underestimated the insurance recovery. Government-program costs are improving, while commercial prices now assume elevated costs persist. Humana's latest results also support Medicare cost stabilization. I forecast a 4% care-delivery operating margin for 2027, below management's longer-term ambitions. Improved quality ratings mainly affect 2028 payments, not next year's earnings.
Valuation
| My estimate or assumption | Value |
|---|
| 2027 consolidated revenue | $460 billion |
| 2027 operating margin | 6.6% |
| 2027 reported earnings per share | $21.86 |
| Core earnings multiple | 17 times |
| Integrated-care franchise premium | 1 additional turn |
| October 2027 target per share | $393.47 |
My segment build assumes gradual insurance repair, stronger care delivery and continued pharmacy earnings; it deducts $4 billion of annual interest, applies 23% tax, subtracts $800 million of minority-owner profits and uses 890 million diluted shares. The extra valuation turn rewards integrated insurance, care and pharmacy capabilities; persistent care losses would remove it. Faster repair could support $500 a share, while renewed deterioration could mean $270; neither outcome has enough evidence to displace hold.
Care-delivery margins determine the upsideAnalyst scenarios, October 13, 2026; other assumptions unchanged; target dollars per share.
Risks
- Claims estimates could flatter recovery: third-quarter savings from earlier estimates contributed $610 million; watch subsequent claims revisions separately from contract-reserve releases.
- Investment spending could overwhelm care-delivery gains; watch fourth-quarter losses and a quantified spending explanation rather than promises of eventual margins.
- Government funding, litigation or costly borrowing could constrain recovery; watch payment rules, legal disclosures and cash available at the parent company, not just consolidated cash.
What would change my mind
First-half 2027 results supporting annual reported earnings above $24 or below $20 a share, without larger reserve benefits, would change this forecast.
